Usage Data is collected automatically and may include Your device's IP address, browser type and version, pages visited, time and date of visit, time spent on pages, and unique device identifiers. When accessing via mobile device, We may also collect device type, operating system, mobile browser type, and other diagnostic data.

We use Cookies and similar tracking technologies to track activity on Our Service. These include Browser Cookies (small files placed on Your Device, which You may instruct Your browser to refuse) and Web Beacons (small electronic files that permit the Company to count users who have visited pages or opened emails). After texting STOP, you will receive a one-time confirmation message, and no further messages will be sent unless you re-opt in.

Carrier Disclaimer

Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your mobile carrier and is not guaranteed by Nokomis Chiropractic and Wellness.

SMS Data — No Sharing for Marketing

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support of services, such as customer service or appointment management platforms, is permitted. All other uses of mobile number data require your express consent. We do not sell, rent, or trade your mobile phone number or SMS opt-in consent data to any third party for their marketing or promotional use. Your mobile number is used solely to deliver messages within our SMS program as described in this policy.

4. Use of Your Personal Data

The Company may use Personal Data to: provide and maintain our Service (including monitoring usage); manage Your Account and registration as a user or patient; contact You by email, telephone, SMS, or other electronic communication regarding appointments, updates, and services; send appointment reminders and health communications via SMS if you have opted in; provide news, special offers, and information about our services; manage Your requests and respond to inquiries; complete business transfers (e.g., a merger, sale, or acquisition of business assets); and perform data analysis and service improvement.

5. Sharing Your Personal Data

We may share Your personal information in the following limited situations: with Service Providers who assist us in operating our website, managing appointments, and delivering SMS communications (contractually obligated to protect your information and not use it for their own marketing); for business transfers; with Our affiliates who are required to honor this Privacy Policy; for legal compliance when required by law or valid government request; and with Your consent. We do not sell or share your personal data — including your mobile phone number or SMS opt-in status — with any third party for marketing or promotional purposes.

The Company will retain Your Personal Data only for as long as necessary for the purposes outlined in this Privacy Policy, including to comply with legal obligations, resolve disputes, and enforce our agreements. Your rights regarding your health information

  • Get a copy of your record — you may inspect and receive a copy of your health record (paper or electronic). We will provide it, usually within 30 days of your request, and may charge a reasonable, cost-based fee.
  • Request a correction — you may ask us to amend health information you believe is incorrect or incomplete. We may deny the request, but we will tell you why in writing.
  • Get an accounting of disclosures — you may request a list of certain disclosures we have made of your health information in the six years before your request.
  • Request restrictions — you may ask us to limit how we use or disclose your information for treatment, payment, or operations. We are not required to agree — except that if you pay for a service in full out of pocket, we must honor your request not to share that information with your health plan.
